Treating Contrast Reactions

Urticaria

  • Benadryl 25-50mg IM/IV
  • ± Epinephrine, Solumedrol

Facial/laryngeal edema

  • Epinephrine (1:1000) 0.1 - 0.3 mg SC
  • Epinephrine (1:10,000) 0.1 mg (1ml) IV Slowly
  • Solumedrol 125 mg IV

Bronchospasm (Wheezing)

  • Oxygen
  • Albuterol nebulized 2.5mg - repeated after 20 minutes if necessary
  • If no improvement:
    • Epinephrine (1:1000) 0.1 - 0.3 mg SC
    • Epinephrine (1:10,000) 0.1 mg (1ml) IV Slowly

Hypotension with tachycardia

  • O2
  • Trendelenberg
  • Volume (Normal saline)
  • Epinephrine (1:10,000) 0.1 mg (1ml) IV
  • Call: HERT

Hypotension with bradycardia (vagal RXN)

  • Trendelenberg
  • O2
  • IV Volume
  • Atropine 0.6mg - 1mg IV. Repeat Atropine up to total dose of 0.04 mg/kg (2 mg in adults)
