family medicine, <\/span><span data-contrast=\"auto\">looking for clues in the brain to explain why some people with schizophrenia have a high rate of comorbid cannabis use disorder.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Park received an <a href=\"https:\/\/www.aan.com\/research\/medical-student-research-scholarship\" target=\"_blank\" rel=\"noopener\">American Academy of Neurology (AAN)\u202fMedical Student Research Scholarship<\/a> to work on <a href=\"https:\/\/geiselmed.dartmouth.edu\/faculty\/facultydb\/view.php\/?uid=423\" target=\"_blank\" rel=\"noopener\">this project with Brunette<\/a><\/span><span data-contrast=\"auto\">, an addiction psychiatrist who has long been involved in research and clinical care of patients with severe mental illnesses and co-occurring substance abuse. <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<figure id=\"attachment_17705\" aria-describedby=\"caption-attachment-17705\" style=\"width: 303px\" class=\"wp-caption alignright\"><a href=\"https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-17705 size-large\" src=\"https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web-303x360.jpg\" alt=\"Soo Hwan Park '25\" width=\"303\" height=\"360\" srcset=\"https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web-303x360.jpg 303w, https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web-109x130.jpg 109w, https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web-46x55.jpg 46w, https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web-580x689.jpg 580w, https:\/\/geiselmed.dartmouth.edu\/news\/wp-content\/uploads\/sites\/2\/2023\/07\/SooHwanPark_web.jpg 645w\" sizes=\"auto, (max-width: 303px) 100vw, 303px\" \/><\/a><figcaption id=\"caption-attachment-17705\" class=\"wp-caption-text\">Soo Hwan (Soo) Park '25<\/figcaption><\/figure>\n<p><span data-contrast=\"auto\">He recently gave an oral presentation on his neuroimaging fMRI research on the potential \"self-medication\" function of cannabis in patients with schizophrenia and comorbid cannabis use disorder for the Behavioral and Cognitive Neurology session during the 2023 Annual AAN Meeting.\u00a0\u00a0\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Cannabis use disorder is common in patients with schizophrenia; nearly 40 percent of patients are heavy cannabis users compared to the healthy population, Park says. \u201cEvidence suggests abnormalities in their brain's reward pathways may be driving their behavior.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cWhen we compared patients with schizophrenia and comorbid cannabis use disorder with healthy controls, we saw abnormal activity in the bilateral caudate as well as the posterior cingulate gyrus\u2014part of the default mode network,\u201d Park says. \u201cAnd abnormal caudate activation has been consistently observed in many fMRI reward function studies in schizophrenia and cannabis use disorder respectively and have been heavily implicated in addiction research.\u201d\u00a0\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Interestingly, Park notes, consumption of the drug is known to worsen symptoms of psychosis and disease prognosis in patients with schizophrenia, yet they continue to use the drug. \u201cWe are exploring possible causes for this to determine why this is the case when it seems to exacerbate their symptoms.\u201d<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Based on their findings the researchers speculate the abnormality may reflect a possible drive for self-medication in these patients. \u201cWhat is also interesting,\u201d Park says, \u201cis that the abnormality in the posterior cingulate cortex, which is one of the hubs of the brain\u2019s default mode network, may be abnormally activated and communicating with the reward network leading to an impaired ability to turn off the default network in patients with schizophrenia. And they may be self-medicating with cannabis because the drug has the potential to normalize the activity in these parts of the brain.\u201d<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Park, Brunette, and the research team are now analyzing the full research data set to see whether smoked cannabis or oral dronabinol\u2014a synthetic form of tetrahydrocannabinol (THC)\u2014can normalize reward network activity. They are testing this hypothesis in the dual-diagnosis group against three control groups: schizophrenia-only, cannabis use disorder-only, and healthy controls.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cI like the nervous system because it models and regulates the intersection between our body and our lived experiences in the external world. Though schizophrenia is primarily considered a psychiatric disorder, both psychiatry and neurology deal with the same organ and the problem of psychosis.
