{"id":1833,"date":"2023-05-03T14:17:03","date_gmt":"2023-05-03T18:17:03","guid":{"rendered":"https:\/\/geiselmed.dartmouth.edu\/medical-education\/?p=1833"},"modified":"2023-05-03T14:18:34","modified_gmt":"2023-05-03T18:18:34","slug":"the-transition-from-medical-student-to-resident-a-qualitative-study-of-new-residents-perspectives-from-codes-to-themes-in-qualitative-research","status":"publish","type":"post","link":"https:\/\/geiselmed.dartmouth.edu\/medical-education\/2023\/05\/03\/the-transition-from-medical-student-to-resident-a-qualitative-study-of-new-residents-perspectives-from-codes-to-themes-in-qualitative-research\/","title":{"rendered":"\u201cThe Transition from Medical Student to Resident:\u00a0A Qualitative Study of New Residents&#8217; Perspectives&#8221; From Codes to Themes in Qualitative Research"},"content":{"rendered":"\n<p>While this article is not as \u201chot off the presses\u201d as the others I\u2019ve chosen, I think it\u2019s timely, given that we are all bidding the fourth year students adieu soon to send them off for what the authors of this piece call a <strong>\u201cprecarious moment in medical training\u201d <\/strong>(p. 1421): <a href=\"https:\/\/www.aamc.org\/about-us\/mission-areas\/medical-education\/transition-residency\/aamc-transition-residency-priorities-and-guiding-principles\">the transition to residency<\/a>. The authors recruited 10 learners who had participated in their residency transition course at the end of medical school and approached them <em>after<\/em> they had started their residency for interviews (between December and April of that first year).&nbsp;<\/p>\n\n\n\n<p>Their central finding is perhaps not earth shattering: building their professional identity\u2013or <strong>\u201cfeeling more like a doctor\u201d <\/strong>as one participant put it\u2013is at the heart of the transition experience (p. 1422). Participants experienced the shift from undergraduate to graduate medical education as abrupt and were particularly flummoxed by \u201cthe omnipresence of nonclinical, administrative tasks\u201d (p. 1424). They were excited to take more responsibility\u2013\u201dYou\u2019re a crucial member of the care team and that is a fundamental change in your self-conception as a health care provider\u201d according to one participant\u201d--but also found the responsibility \u201cscary\u201d at times (p. 1424). Also (again, no spoiler alert needed here), they were challenged with work-life balance, feeling <strong>\u201ctremendous tensions and stress\u201d<\/strong> (p. 1425).&nbsp;<\/p>\n\n\n\n<p>While the themes the authors found are not necessarily surprising, the way they share these individuals\u2019 <em>particular<\/em> experiences and voices through these themes offers fresh insight into what it feels like to make the transition to residency. Moreover, as the cool visual they created shows, they offer a really interesting and useful metaphor as a model for this transition:<strong> a cell<\/strong>. They describe it this way:&nbsp;<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\"><p><em>In this model, the nucleus of the cell represents the learner experience of professional<\/em> <em>identity development, characterized by ambivalence around autonomy and responsibility<\/em> <em>and the learner\u2019s striving to maintain learning while balancing and integrating the residency experience into life. Intracellular elements\u2014illustrated as mitochondria, nucleoli, and the Golgi apparatus\u2014highlight the dynamic interactions in the environment between the new resident and patients, colleagues, supervisors, and other medical professionals that lead to cascades of learning and development. A porous nuclear membrane highlights how the new resident influences and is influenced by the surrounding environment. <\/em>(p. 1425)<\/p><\/blockquote>\n\n\n\n<p>But, as you might suspect, I didn\u2019t <em>just<\/em> choose this for the conceptual model: there\u2019s a methodological point I want to make, particularly for those who are <strong>getting their feet wet with qualitative research<\/strong>: there is a difference between a \u201ccategory\u201d or \u201ccode\u201d and a \u201ctheme.\u201d Here\u2019s a little diagram to show what the process looked like for them:<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/geiselmed.dartmouth.edu\/medical-education\/wp-content\/uploads\/sites\/13\/2023\/05\/coding.jpg\" alt=\"coding diagram\" class=\"wp-image-1839\" width=\"624\" height=\"371\" srcset=\"https:\/\/geiselmed.dartmouth.edu\/medical-education\/wp-content\/uploads\/sites\/13\/2023\/05\/coding.jpg 624w, https:\/\/geiselmed.dartmouth.edu\/medical-education\/wp-content\/uploads\/sites\/13\/2023\/05\/coding-300x178.jpg 300w, https:\/\/geiselmed.dartmouth.edu\/medical-education\/wp-content\/uploads\/sites\/13\/2023\/05\/coding-93x55.jpg 93w, https:\/\/geiselmed.dartmouth.edu\/medical-education\/wp-content\/uploads\/sites\/13\/2023\/05\/coding-580x345.jpg 580w\" sizes=\"auto, (max-width: 624px) 100vw, 624px\" \/><\/figure>\n\n\n\n<p>In the first step, they generated a bunch of codes with an eye towards their research question. Then they grouped these codes into categories. Then (and this is the critical step), they looked across all of the categories and tried to pull out some \u201cbig picture\u201d themes to answer their research question. For instance, two <strong><em>codes<\/em><\/strong><strong> <\/strong>were \u201cpersonal and professional balance\u201d and \u201cnonclinical responsibility.\u201d These each ended up in two different <strong><em>categories<\/em><\/strong>, \u201cpersonal balance\u201d and \u201ccontext of learning\u201d respectively. Then these two categories (and the codes within them) <em>both<\/em> contributed to the <strong><em>theme<\/em><\/strong>, \u201cabrupt change in educational environment from medical school to residency.\u201d<\/p>\n\n\n\n<p>As you can see, this process involves both <strong>subjectivity<\/strong> and <strong>rigor<\/strong>.
