Watching the whole process through their eyes and through the eyes of my colleagues got me curious about the process and how it works, which led me to this very interesting recent article by Kaitlyn Kaiser, Heather Lench (both in psychology at Texas A&amp;M) and Linda Levine (in Neurobiology at UC Irvine; see full citation below). They were curious about the emotions right after the match and how they lined up with emotions a few months into residency, so they did a survey (total <em>n<\/em> = 182) at three timepoints:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Time 1: <\/strong>A week before Match Day<\/li><li><strong>Time 2: <\/strong>A week after Match Day<\/li><li><strong>Time 3: <\/strong>October of PGY-1<\/li><\/ul>\n\n\n\n<p>I love the design of this study because they used <em>both<\/em> pre-existing survey measures that had been validated on medical students (of happiness, excitement, life satisfaction, stress, burnout) <em>and<\/em> their own measure, which they created from <em>survey participants\u2019 responses<\/em>. (just for fun, see <a href=\"https:\/\/keylimepodcast.libsyn.com\/305-six-survey-design-principles\">here <\/a>for an awesome podcast on survey design!) At Time 1, they asked participants to list the \u201cfive most important factors they considered when making their rank list\u201d (p. 2). The research team then did <a href=\"https:\/\/www.plymouth.ac.uk\/uploads\/production\/document\/path\/18\/18247\/Kiger_and_Varpio__2020__Thematic_analysis_of_qualitative_data_AMEE_Guide_No_131.pdf\">thematic analysis<\/a> to code these responses into 9 categories:<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li><strong>Location<\/strong><\/li><li><strong>Prestige <\/strong>(i.e., program reputation)<\/li><li>Post-residency <strong>opportunity <\/strong>(career paths of recent graduates, quality of fellowship prep)<\/li><li>Program <strong>culture <\/strong>(e.g., \u201cgut feelings\u201d about program and visit, current resident morale, rapport with leadership, work\/life balance, size of patient caseload, cultural\/racial\/ethnic\/gender diversity)<\/li><li><strong>Relationship <\/strong>(i.e., support network in area and needs of your partner or family)<\/li><li><strong>Training <\/strong>(e.g., quality of training, diversity of patient problems\/procedures, research opportunities)<\/li><li><strong>Finances <\/strong>(i.e., cost of living, salary, benefits)<\/li><li><strong>Likelihood <\/strong>of matching<\/li><li><strong>Other <\/strong>(e.g., training length, schedule, characteristics of patient population)<\/li><\/ol>\n\n\n\n<p>Then they used these categories as <a href=\"https:\/\/stats.oarc.ucla.edu\/other\/mult-pkg\/whatstat\/what-is-the-difference-between-categorical-ordinal-and-interval-variables\/\"><em>categorical<\/em> items<\/a> at Times 2 and 3. Cool, right? But here\u2019s the rub: 3 time points and a bunch of different measures makes for a complicated study\u2013you can bet they had a good stats person on board to help them figure out <a href=\"https:\/\/courses.washington.edu\/psy524a\/_book\/two-factor-anova.html\">when to run an ANOVA across multiple categories (\u201comnibus\u201d--but not a mode of transportation, or at least not to a destination in real life\u2026), when to do independent comparisons<\/a>, and when to run a <a href=\"https:\/\/bmcmededuc.biomedcentral.com\/articles\/10.1186\/s12909-020-02295-y\">regression<\/a>.&nbsp;<\/p>\n\n\n\n<p>But this careful and complicated design yielded cool results:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>At <strong>Time 1<\/strong>, Location, Training (particularly, quality), and Program Culture (particularly, gut feelings about goodness of fit) were the most important factors.<\/li><li>At <strong>Time 2, <\/strong>the higher the rank of the match you got, the <em>happier, more excited, more satisfied, <\/em>and <em>less stressed<\/em> you were.<\/li><li>At <strong>Time 3<\/strong>, these significant differences disappeared.<\/li><\/ul>\n\n\n\n<p>In other words, a few months into residency, it really didn\u2019t matter if you got your top choice. What <em>did<\/em> matter was program culture and whether participants felt they fit into that culture.<\/p>\n\n\n\n<p>I feel like this has huge implications for how we counsel our learners here at Geisel: first, if you didn\u2019t get your top ranked match, it\u2019s okay! Second, for our 2024 learners and beyond, pay attention to program culture when you do your ranking: things like \u201cfeelings about the program, morale and rapport, life balance, caseloads and call schedules\u201d (p. 6).<\/p>\n\n\n\n<p>So, of course I\u2019m going to end by pointing out <strong>research opportunities<\/strong>: the authors suggest continuing the study of emotions and perceptions of program culture throughout residency (some research for you GME folks out there!). But it also made <em>me<\/em> wonder about the <em>process<\/em> medical students go through to make their rank order lists: who do they confide in? How does their \u201c<a href=\"https:\/\/indigenousinitiatives.ctlt.ubc.ca\/classroom-climate\/positionality-and-intersectionality\/#:~:text=Positionality%20refers%20to%20the%20how,identities%20and%20access%20in%20society.\">positionality<\/a>\u201d make a difference, like their race, gender, disability status, or sexual or gender minority status?
