A logic model is just that, a model, usually on one-page, that shows the “if-then” relationships between the inputs and resources (personnel, funding, technology or material supports) you plan to invest, the strategies or activities you plan to do with your target populations, and the results and outcomes we will help you measure to assess success and ultimate impact.
