Referring Physicians
The Dartmouth-Hitchcock Sleep Disorders Center is a sleep consultation service that accepts patients for evaluation upon referral from their physicians. Patients who contact the Sleep Disorders Center directly are encouraged to initially review their sleep-related issue with their primary care physician or health care provider.
If you are a health care provider and have a patient with a sleep-related issue whom you would like to refer for consultation by one of our sleep specialists, please complete the Sleep Disorders Center referral form or contact the DHMC Physician Connection Line at 1-866-DHMC-DOC (346-2362). Specially trained representatives are available Monday through Friday, 8am to 6pm, to answer your call. This service and other programs specifically designed to assist referring providers are offered through the Department of Referring Physician Services.
