PPHP - A Team Approach
A multidisciplinary team comprising of the psychiatrist, nurse and mental health therapist meets with the patient upon admission to the Psychiatric Partial Hospitalization Program (PPHP) to develop a preliminary evaluation of the patient and identify how the program can be of help. The mental health therapist facilitates the skill-based groups and the nurse meets daily with the patient individually to address progress toward goals. The social worker meets with the patient individually to complete a psycho-social evaluation that aids the treatment of the patient. The social worker help connect the patient to resources to meet identified psycho-social needs. The occupational therapist facilitates activity-based groups focused on experiential learning to increase self-awareness. Together these professionals work with participants to develop treatment goals and plans. The team works with patients, their families and the existing treatment providers in the community to plan for continued care. Aftercare plans focus on establishing a personalized strategy that prevents relapse and supports recovery.
