In Association WithPPHP - Making a Referral
The following information is necessary when making a referral to PPHP:
- Current behaviors requiring admission
- Assessment of the current level of dangerousness to self or others
- Past acts of dangerousness to self or others
- Precipitant to current episode
- Diagnosis
- Medical stability and cognitive ability to fully participate
- Significant medical history
- History of substance abuse and a willingness to abstain from substances while in the program
- Past psychiatric care including both inpatient and outpatient treatment
- Current medication and current dosages
- Referring clinician's anticipated goals for treatment
