PPHP - Admission

Potential patients are referred by a mental health provider in the community.
The referring provider calls the Psychiatric Access Services at (603) 650-7500 with the patient information. The Psychiatry Access intake clinician determines if the patient meets criteria for admission, and gathers demographic information and verifies the patient's health insurance benefits.
The referrer calls the health insurance plan to obtain pre-certification if directed by the Psychiatry Access intake clinician.
On day of admission, patient goes to the Admissions Office by 8:30am. When completed the patient can proceed to the Partial Psychiatric Hospitalization Program on level 2.
The patient meets with the treatment team for an admission interview.
The treatment plan and nursing evaluation are completed by the patient together with the nurse.
The mental health therapist gives the patient an orientation to the program.
The physician gives the patient a physical exam, unless patient declines or has had a recent physical exam.
The patient joins other participating patients in the first group of the day.
The program ends at 4:30pm. Patients can expect to leave the program for the day between 4:30 and 5pm.
