PPHP - Admission
- Potential patients are referred by a mental health provider in the community.
- The referring provider calls the Psychiatric Access Services at (603) 650-7500 with the patient information. The Psychiatry Access intake clinician determines if the patient meets criteria for admission, and gathers demographic information and verifies the patient's health insurance benefits.
- The referrer calls the health insurance plan to obtain pre-certification if directed by the Psychiatry Access intake clinician.
- On day of admission, patient goes to the Admissions Office by 8:30am. When completed the patient can proceed to the Partial Psychiatric Hospitalization Program on level 2.
- The patient meets with the treatment team for an admission interview.
- The treatment plan and nursing evaluation are completed by the patient together with the nurse.
- The mental health therapist gives the patient an orientation to the program.
- The physician gives the patient a physical exam, unless patient declines or has had a recent physical exam.
- The patient joins other participating patients in the first group of the day.
- The program ends at 4:30pm. Patients can expect to leave the program for the day between 4:30 and 5pm.
